Multiple Teams · TOA Announces Free Physical Date

Tennessee Orthopaedic Alliance and physicians/staff from Middle Tennessee Medical Center will again offer free pre-participation exams for prospective Rutherford County middle and high school student athletes on Saturday, April 27, 2019.  To make this day go as smooth as possible, please note the following:

Athletes must present all appropriate forms, completed and signed by parent.  There are 3 forms needed to be completed for you to receive an exam:

  1. History Form
  2. Consent for Athletic Participation & Medical Care Form
  3. Blank Physical Form with the Athletes Name
  • You can obtain your forms two different ways:
  1. Update all your forms on Dragonfly and print them when finished and bring to TOA with you. This step will save you time as you will not have to fill out forms by hand AND then go update on Dragonfly.
    • Go to your Dragonfly account and update your History Form, Consent for Athletic Participation & Medical Care Form. (Along with all other forms on Dragonfly)
    • Print the History Form, Consent for Athletic Participation & Medical Care Form off Dragonfly.
    • Print attached Blank Physical Form
    • Bring All Signed Forms with you to TOA. (these forms will have your electronic signature on them)
  2. Print the attached History Form, Consent for Athletic Participation & Medical Care Form and Blank Physical Form.
    • Fill out and sign History Form by hand.
    • Fill out and sign Consent for Athletic Participation & Medical Care Form by hand.
    • Print Blank Physical Form and put the athlete’s name on the form.
    • Bring All Signed Forms with you to TOA.
  • Athletes without completed and signed forms will not be eligible to receive an exam.
  • Please have all pages filled out with all required parent signatures before the time of entrance.
  • If you have not created a DragonFly Max account, instructional information will be on the Siegel Athletic website,
  • Printers will not be available the day of exams.
  • This will be the only opportunity for athletes to receive these exams – no make-up will be done.
  • Athletes are not required to attend these exams, but a physical is required for participation in sport.
  • Physicals for the 2019-2020 school year have to be dated on or after April 15, 2019 and will be good through June 20, 2020
  • Please avoid any caffeine/energy Blood pressure will be taken and athlete may not be cleared.
  • Athletes should dress in shorts/t-shirts to allow physicians to evaluate extremities as needed.

  • Parents are welcome to accompany their children throughout the entire procedure.
  • Exams will be complimentary; however, donations will be accepted Proceeds will be used to purchase medical equipment used by the Rutherford County Athletic Trainers.
  • Please note that some medical problems detected during physical exam may require clearance from the athlete’s physician. Parents may wish to schedule a complete physical evaluation from their physician.
  • Each student is responsible for delivery of forms to their coach – copies will not be available
